Adolescent inpatient program discharge planning to encourage lasting sobriety
We also complete a thorough discharge plan for each adolescent who receives care at Options Behavioral Health. By the time a young person is ready to transition home or to his or her next level of care, we will have produced a comprehensive discharge plan that will detail his or her progress while in treatment, will note issues of concern or areas that merit continued focus, and will identify the professional referrals and other sources of support that will be necessary for the young person to continue the progress that he or she made while in our care.
